HIPAA Privacy |

According to
the Department of Health and Human
Services (HHS), the HIPAA Privacy Rule is the first federal law created to
contend with privacy security and is intended to protect patient privacy. The
rule holds covered entities accountable for its uses and disclosures of
protected health information. The rule also gives health plan members greater
control over their protected health information by granting members access to
their health information, giving members the opportunity to object to the uses
and disclosures of their health information, and allowing members to request
restrictions and amendments to their health information. Furthermore, through
the requirement of a privacy notice, members will have a better understanding
of how their protected health information may be used and disclosed.
Protected health information (PHI) is individually identifiable health
information, including demographic information, collected from you or created
or received by a health care provider, a health plan, your employer (when
functioning on behalf of the group health plan), or a health care clearinghouse
and that relates to: (i) your past, present, or future physical or mental
health or condition; (ii) the provision of health care to you; or (iii) the
past, present, or future payment for the provision of health care to you.
We may use your PHI for payment and healthcare operations purposes. We
may disclose your protected health information to you, HHS, the plan sponsor
for plan-related activities, and as permitted or required by law. We may also
disclose minimal PHI to your family members or friends who are involved in your
health care. We may not use or disclose your PHI for any other reason unless
authorized by you.
